exactly what it looks like, an oem derbi axle nut. these came on pretty much every derbi moped ever.

12mm ID - 20mm hex - 1.5mm thread - 9mm thick

also works on the piston port/pyramid output shaft as the nut to hold the sprocket on IF you have the threaded type. there is also the circlip type which of course this will not work for!!
